APSEA's Lifestyle Change Program is designed to support individuals with elevated blood pressure and blood glucose levels, offering consistent follow-ups to monitor and improve health.
Our program includes:
One-on-one coaching sessions with a CDC lifestyle coach, trained APSEA volunteers, and Dr. Aayush Visaria, MD, MPH, as needed.
Monthly virtual sessions on preventive health topics (diet, physical activity, etc.) for 6 months.
Monthly screenings including blood pressure, fasting blood sugar, A1c testing, BMI, waist circumference, body fat, and heart rate.
Screenings can be conducted at your home or an APSEA screening location.
Weekly check-ins with an APSEA volunteer to keep you on track and answer health-related questions.
The program is free, and you are welcome to join or leave at any time.

The National Diabetes Prevention Program (National DPP) brings together public and private organizations to help prevent or delay type 2 diabetes and hypertension. This partnership makes it easier for individuals at risk to access evidence-based lifestyle change programs, empowering them to reduce their risk and improve their health.
